Daffodil Standard for older people in care homes

The COVID-19 pandemic has highlighted the need for personalised care and support planning for people residing in a care home more than ever. Many of these people are living with frailty, dementia, multimorbidity or another specific life limiting illness

This Daffodil Standard combines all the existing Daffodil Standards as they apply to care homes, in a user-friendly practical format which can be applied directly into practice.

It has been designed specifically to help General Practice care for people in care homes and support the homes to provide the best possible care for their residents.

Levels to complete

The standard will use a simple three level structure with the first two levels available now. The third is due early 2021. This helps practices prioritise and work towards improvement from whatever is their baseline.

Level 1 – Core Essentials – Internal practice systems to enable consistency of care

This looks at systems within the practice although including how you involve your multidisciplinary team (MDT) some of who maybe external to the practice. Most practices either on their own or along with members or their Primary Care Network, GP Cluster or Federation should be able to aim for level 1.

Level 2 – Enhanced - Improving communication, shared planning and compassionate care

This builds on embedding ongoing continuous quality improvement, sharing learning and ideas within your team and the wider MDT. It encourages innovation, generation and testing of ideas and learning from death, each other and the wider team.
