The Fairer Practice Toolkit Webinar 3 - theme is homelessness and access to care.

Learning objectives for the session: • Understand how homelessness affects access, continuity, and health outcomes • Understand how to use EHR population analytics e.g. EMIS, SystmOne, or dashboards to identify people experiencing homelessness or unstable housing as a cohort • See a practical demonstration of how to use relevant data to make sense of population health data to reveal inequalities you can act on e.g create a clear workable list of patients who would benefit most from targeted support - subject to discussions with the speaker but this would be really valuable insight I suspect • Leave with a clear step by step quality improvement process they can implement to take action to improve the care experience of people with homelessness
